Kokoro Healing Collective is bound by the Australian Privacy Principles (APPs) contained in the Privacy Act 1988 (Cth) (Privacy Act). The APPs are designed to protect the confidentiality of information and the privacy of individuals, by regulating the way personal information is collected, used, disclosed and managed. This policy explains the types of personal information that we may collect and hold, how that information is used and with whom the information is shared. It also sets out how you can contact us if you have any queries or concerns about this information. Why do we collect personal information? We collect personal information about website users to understand and meet their needs, to conduct our activities and meet legal obligations. Clients’ files are held in a secure location on the premises and/or electrinically (on the practitioners booking site). Practioners may also be consulting with their supervisors about the client's sessions from time to time. However, client's details are unidentified to protect the client's privacy. These communications are confidential and the purpose of the supervisions is to enhance the quality of care clients receive. What personal information do we collect? For example, we collect your contact details for appointment confirmation and rescheduling. This may include name, contact details such as address, phone number, date of birth as well as details of issues which you may discuss with a practitioner. As professional health allied practitioners, we are oblidged to collect your data for treatment purposes. We use a variety of individual Client Intake forms. Each practitioner may have a different form suited for their modality.
HOW WE COLLECT AND HOLD IT We may collect and hold your personal information when you request or acquire a product or service from us, when you register for a workshop or support group, complete a survey or questionnaire, participate in our services or when you communicate with us through this website or by e-mail, telephone or other messaging services. We hold personal information in paper form and electronic form.
HOW WE USE YOUR PERSONAL INFORMATION The information is gathered in order to document what happens during sessions and enables us to provide a relevant and informed service that supports you. We may collects certain information for statistical and professional purposes. However, this information may not considered personal or sensitive information. Information is only considered to be personal information if the identity of the individual is apparent or can be reasonably ascertained from the information. Your information will be used in order to compile statistics such as the number of clients we see on a monthly basis, the days and times we see clients, the ages of clients, etc. This information may be used to manage or develop our services. CONFIDENTIALITY Disclosure of personal information Kokoro Healing Collective will not disclose the personal information you provide us with to any third party individuals or organisations without your expressed permission. We may disclose your personal information, but only if it is required, or authorised by law, or where disclosure is necessary to prevent a threat to life (to yourself or others), health or safety, or where we are otherwise permitted by the Privacy Act.
